Treatment of midshaft clavicle fracture with Intramedullary Nailing versus Plate

Introduction: Titanium elastic nail osteosynthesis has increasingly been used as a minimally invasive
alternative for displaced midshaft clavicle fracture management (AO/OTA 15.2A). However, its use
remains controversial because of its complication profile.
Methods: A retrospective comparative study of 99 surgically managed displaced midshaft clavicle
fracture with elastic nails or plate fixation in four hospitals was performed. Functional result was
Functional result was calculated using the Constant and Quick-DASH scores.
Results: Sixty-one osteosynthesis with elastic nails and 38 plate fixations were reviewed. Osteosynthesis with elastic nails shorter surgery time minutes; smaller scar, and higher Constant score than
plate fixation. Alterations in consolidation and scar numbness were higher in plate fixation. However, irritative symptoms and prominence of material were higher in osteosynthesis with elastic nails.
Conclusion: Intramedullary fixation with elastic nails for displaced midshaft clavicle fractures offers a minimally invasive alternative with better aesthetic and functional results, and fewer major
complications, compared to plate fixation.
